Patient Centered Medical Home Blog
It looks like earlier this year a new blog/Web site was launched for discussion and collaboration on EHR, PHR, and Patient Portals. You can find it at www.ehrphrpatientportal.blogspot.com. We can’t comment yet on its value, but it may be something to check out as it develops over time.
However, it doesn’t appear right now to focus on what we feel is the true definition of the patient-centered medical home. The PCMH isn’t all about technology. The foundation is the primary care physician and central site capabilities (care management, wellness programs, etc.) where all the patient’s records can be accessed (or as many as possible). That primary care site is just as important as the connectivity, because that is where all medical information is processed, known, and evaluated, and the care is coordinated for the best possible outcome. Sure, having connectivity makes this happen more easily, more efficiently, and more effectively. But without the center, there is nothing for the other components to connect with. You have to have the “on-site” component or the off-site data can’t be processed even if it is available.

This is a very informative site. Thank you for commenting on our site. I am in complete agreement that there needs to be a “center” or core in any Medical Home. My goal is to show that solo and small practices can coordinate information and services for patients as well as most large groups thanks to the connectivity provided by evolving software, hardware and broadband.
