In business since 2008. They currently serve primarily the Midwest (Indiana, Ohio, Illinois, Michigan, Wisconsin and Pennsylvania). As of the fall of 2015, they have 30 clinics with primary care coverage on a management fee basis (all staff are employed by Activate) and have a partnership with Walmart to reduce drug costs. Behavioral support programs are a focus to reduce costs and improve individual health. They do on-site, near site, and shared site facilities.
