American Health Network (AHN) is an independent, physician-owned medical group with over 200 physicians and 70 locations in Indiana and Ohio. Expanding its services in 2013, AHN opened its first onsite clinic through its Employer Health division with a commitment to innovation and continuous growth. As of April, 2016, AHN operates 6 employer-sponsored on-site clinics as well as 11 onsite RN health coaching contracts in Indiana with plans to expand in the Midwest over the next few years.
They state that population health magement is one of their strengths using a robust electronic infrastructure to drive results. AHN believes its role is “to not only help employers improve the health of their populations but to help them become a better place to work.” Services include RN Health Coaching, Healthcare Analytics, Customized Outreach Programs, Chronic Condition Management, Wellness Incentive Tracking, Telehealth and Telemedicine. All staff are employed by AHN; their practitioner focus is to take the time to get to know participants in order to optimize engagement and outcomes.
