Premise Health was formed in 2014 through the merger of Take Care Employer Solutions and CHS Health Services.  The organization is one of the largest in the industry with a long history of providing services to employers in both the onsite primary care market and occupational health.

Premise offerings focus on the patient-centered medical home; primary care; occupational health; and pharmacy with everything in between.  They have more than 500 worksite health centers in the United States encompassing nearly every industry; including many in the Fortune 1000 list.

They have worked hard to obtain accreditation from AAAHC for many of their sites, making them accountable for their operations to an independent third party accrediting body.

UPDATE:  February, 2016.  Premise announced the acquisition of existing provider, IMWell Health.  This gives Premise additional capacity and talent in serving the mid-sized employer market as well as one with experience in multi-employer sites.  IMWell was been around since 2004 and has been listed separately on this web site previously.

UPDATE:  July, 2016, Premise announced the acquisition of TransformHealthRx.  TransformHealthRx has a presence in five Midwestern states, with 17 sites at this time, and gives Premise strength and experience in providing services to municipalities as well as private employers.

UPDATE:  July, 2018, Water Street sold its shares in Premise to OMERS Private Equity.  They now provide services in 44 states, Puerto Rico and Guam, with 4,500 employees and 600 sites.
